It is a good three of four years since I looked at a mod 7 paper, however fault trees always seemed to be a 'near' sure thing for an exam question. Issue 142 February 2009 of IRSE News includes a Technical Tip on fault trees by one of our Younger Members. See the aricle on page 24 & 25, it is worth a look.

Don't worry, you are not the only one still waiting; I had an email from someone in Glasgow a few hours ago........

I haven't got a soft copy but I know the author, so will get in touch with him for you.

In the meantime you may care to look at the signal question that we did at the 2007 GM Rail event under the Module 5 heading that gives an example in the form which you could do in the exam.

Please see attached - hope it is of some use. For IRSE exam purposes, the example PJW has referred you to is probably more beneficial so I'd strongly recommend you look at that. If you have any questions, let me know.

Here is a further explanation of Fault Tree Analysis which might be useful for module 5 questions or perhaps even more module 7 questions.
Whereas the fomer would be focussed on using the technique to display knowledge of a piece of equipment and its potential faults / failure modes, a mod 7 question is likely to look at a higher level at a more complex system and expect a more detailed understanding of the technique itself.
